The tasks in section “B” are completely different. They engage the right hemisphere and require that you form a certain picture in your head, and then the left hemisphere translates this picture into logical instructions.

You can easily count the numbers from 26 to 0 - partly due to mechanical thinking, and partly due to logic. To do this, it is enough to use one left hemisphere. However, most people cannot recite all the letters of the alphabet in reverse order because the alphabet has been learned mechanically and there is no logic to it. A different approach is needed here.

There are several simple techniques that allow you to engage not only the left, but also the right hemisphere of the brain:

fMRI, as a non-invasive method, allows one to trace intra- and interhemispheric reorganization of the speech system in case of damage to the left hemisphere during speech restoration and is widely used in patients with strokes. fMRI or PET - studies of patients after removal of a tumor from the speech-dominant hemisphere are practically absent. Not all tumors of the left hemisphere, with their slow growth and possibilities of brain plasticity, are accompanied by the appearance of aphasia. Consequently, these studies could answer the question: is the emergence of right-sided foci of activation during speech tasks always observed in the presence of aphasia, and, conversely, if aphasia, even severe, has developed, is the right hemisphere always activated? And what is the possible functional significance of the activation of the right-sided homologs of speech areas?

The purpose of this work was to study, using fMRI, the interhemispheric reorganization of the functional speech system in patients with gliomas of the left hemisphere in comparison with the dynamics of speech disorders.

In total, the condition of 20 people with gliomas of the left hemisphere of the brain was monitored over time using fMRI and a comprehensive neuropsychological study. In 12 patients the tumor was localized in the frontal lobe, in 8 patients - in the temporal lobe. Patients were examined before surgery, 3-10 days after tumor removal, then 3 months, 5-7 months and 1 year after surgery.

Neuropsychological research using the method of A. R. Luria and fMRI were carried out on the same day. It was considered (Glozman Zh. M., 2012) that 0 points - normal speech functions, 1 point - mild speech impairment, 2 points - moderate, 3 points - severe.

The MRI examination was performed on a SignaHDxt (GE) 3.0 T magnetic resonance imaging scanner. The NordicNeuroLabActiva attachment was used to conduct fMRI. During the study, a standard speech block paradigm was used; 5 tasks were presented. To improve the accuracy of the data, high processing thresholds of p < 0.001 were used instead of group analysis on a case-by-case basis.

The leading hand was determined by self-assessment and the M. Annett questionnaire (Bizyuk, 2005). All patients were right-handed; only 1 patient with a temporo-insular tumor had signs of left-handedness (sometimes he did some of his housework with his left hand).

Speech disorders before surgery were detected in 6 patients out of 19 to a mild to moderate degree (2 with frontal lobe tumors, 4 with temporal lobe tumors).

Before surgery, in most patients, according to fMRI data, only left-sided localization of speech zones was noted. However, in 4 patients, bilateral activation was already noted before surgery: on the left - Broca's area and its homologue, on the right - according to data from various speech tasks. These were two patients with tumor recurrence and aphasia and two more patients with primary gliomas without speech disorders, but with extensive tumors of the fronto-insular region. It should be emphasized that activation of right-sided homologs of Broca's area was detected in patients without speech impairments in the presence of large, long-growing Gr II tumors.

After the operation, the appearance of speech impairments of varying degrees was noted in 10 patients (and in 6 patients it already existed before the operation), that is, after the operation, speech remained normal in only 4 patients out of 20. fMRI and neuropsychological examination in the early postoperative period were performed in 11 patients. In all patients, even despite the appearance of severe speech disorders in some cases, foci of activation remained in the speech zones in the left hemisphere in response to various speech loads.

Thus, during an fMRI study immediately after surgery, the pattern of activation of speech zones more often than not remained stable, left-sided foci of activation of speech zones were preserved, and no additional activation zones were identified in the left hemisphere at our statistical processing thresholds based on fMRI data. In 2 cases, activation zones homologous to speech appeared in the right hemisphere in patients with extensive tumors.

Upon further monitoring of the patients' condition over time in comparison with fMRI data 3.5-7 months after tumor removal, the following picture was discovered. Aphasia gradually regressed in all patients, even with malignant gliomas. Moreover, different aspects of speech function could be restored to varying degrees. Most often, changes in fMRI were noted in patients when examined after 5-8 months, but sometimes they were detected when patients were examined as early as 3 months after surgery. First of all, the changes concerned the appearance of activation of zones of the right hemisphere, homologous to speech ones. Activation of speech areas of the left hemisphere remained stable throughout the observation period in all patients. We did not observe the emergence of new activation zones in response to speech loads at our statistical processing thresholds.

When analyzing fMRI data over time, two groups of patients could be distinguished:

1. Patients in whom, during the entire observation period (up to 1 year or more), there was no connection of activation of the right-sided homologs of Broca’s and Wernicke’s areas in all the tests we used, while the localization of activation of the left-sided speech areas also remained stable - 4 patients. Moreover, in 3 of them, aphasia after surgery was moderate or severe, in 1 patient speech was preserved.

2. Patients in whom left-sided activation of speech zones was accompanied by activation of right-side homologs of speech zones according to at least some tests—16 people. In 4 of them, aphasia after surgery was severe, in another 9 it was moderate or mild. And what is very important, in 3 people the speech remained normal. A year or more later, 12 patients were studied. At the same time, in all patients, foci of activation in the left hemisphere remained in the same places, and in almost all of them, foci of activation in the right hemisphere remained in response to speech load. That is, in patients with speech impairments (even moderate and severe) there may have been no shifts in the activation of speech areas according to fMRI data during speech loads over long periods of observation. At the same time, activation of speech homologues of the right hemisphere (Broca's area and Wernicke's area) was also observed in patients who did not have aphasia after surgery. It is important that this activation was detected only after surgery. All this may indicate individual characteristics of the restructuring of the speech functional system.

Thus, foci of activation of the right hemisphere in response to speech loads according to fMRI and PET data have been identified in both strokes and brain tumors. According to the literature, the emergence of right-sided activation of homologs of speech areas, identified using fMRI or PET in response to speech loads, along with the appearance of additional foci of activation in the left hemisphere, are considered compensatory processes in the restoration of speech functions impaired due to strokes and brain tumors (Meyer PT , 2003; Winhuisen L., 2005; Thiel, 2004, 2006a; Crosson B. 2007; Anglade C., 2014; Winhuisen L., 2007; van Oers CAMM, 2010]. Most of these authors indicate an unfavorable prognosis for speech restoration with connecting the activation of the right hemisphere to perform speech tests. However, in the case of brain tumors, the pathogenesis of brain destruction is fundamentally different - the tumor grows gradually, during surgery they try to remove mostly its mass with minimal damage to brain tissue.Nevertheless, in some cases, in the complete absence of aphasia, we have detected the appearance of activation of zones of the right hemisphere - in some patients already before surgery, in other patients only after surgery.

Currently, there are two fundamentally different approaches to assessing the significance of activation of areas of the right hemisphere and, therefore, to the possibilities of influencing the right hemisphere in order to restore speech in aphasia.

The first, characteristic of domestic rehabilitation, is the use of the capabilities of the right hemisphere (and its activation) with the aim of including it in speech restoration and subsequent disinhibition of the left hemisphere. This makes sense for various aphasias, especially their severe forms. For example, disinhibiting singing of a patient with gross motor aphasia, rhythmic and melodic techniques, emotional exclamations “oh!”, “oh!”, stimulation of understanding of situational and everyday speech with temporal aphasia, recognition and arrangement of ideogram signatures under drawings (Tsvetkova L. S. ., 1985). However, the capabilities of the right hemisphere in terms of speech restoration are limited (Gainotti G., 1993, Bidelman, 2016).

Studies of patients with “split brain” by R. Sperry, M. Gazzaniga, E. Zaidel (1978), as well as patients after unilateral electroconvulsive therapy by L. Ya. Balonov, V. L. Deglin, T. V Chernigovskaya (2002) showed that the right hemisphere does not have phonetic coding. Therefore, understanding of oral and written speech in the right hemisphere is carried out not on the basis of phonetic analysis, but by the method of integrated, gestalt comparison of the sound and graphic image of a word with memory standards and subsequent association of this image with meaning. It has been suggested that the right hemisphere may also be involved in understanding metaphors (Gianotti G., 2016).

There is an opposite point of view - about reciprocal relationships between the hemispheres, the “interhemispheric competitive model” (Takeuchi N., Izumi Sl, 2017), when the weakening of the functions of one hemisphere leads to a reflex strengthening of the functions of the other. On this basis, inactivation of parts of the right hemisphere homologous to the speech zones of the left hemisphere using repeated transcranial magnetic stimulation (rTMS) has now become very actively used to restore speech function (Barwood CH, 2011; Turkeltaub P., 2011; Heiss WD, 2016). At the same time, according to A. Thiel (2006), there is a reduction in transcallosal inhibition of the left hemisphere and, as a result, a disinhibition of its functions with an improvement in speech activity. However, this raises many questions. There is no obvious evidence of reciprocal relationships between the hemispheres, for example, with paresis of one arm there is no the function of the other does not improve (except against the background of forced social adaptation). On the contrary, with unilateral hemiplegia, the patient is actively forced to act with healthy limbs, and for the development of speech in a small child, the development of fine motor skills of both hands is stimulated. With right-sided (frontal) lesions, indeed, it can there may be disinhibition of speech, but only within the framework of the patient’s general disinhibition (euphoria, uncriticality, etc.). In addition, studies suggesting inactivation of the right hemisphere using rTMC, as a rule, present few observations, and the treatment can be “stretched” over time, which can also provide speech restoration.

In conclusion, our data show the individual degree of involvement of the right hemisphere in speech functions. Despite the limited possibilities for the participation of the right hemisphere in ensuring speech function, it is impossible to neglect them when restoring speech. The hypothesis about the reciprocal relationships of the hemispheres in ensuring the function of speech, in our opinion, today seems unproven practically and insufficiently supported theoretically.

Use emotions

There is nothing wrong with emotions. They exist, and at the same time they can have an impact much stronger than logical constructions. The combination of emotions and logic can sweep away all obstacles in its path. Television advertising becomes much more effective when it appeals not only to logic, but also to emotions. For example, one of the commercials dedicated to car tires especially emphasizes their role in ensuring safety. The advertisement uses an image of a huge tire with a child sitting in it - this is a combination of logic and emotion.

Reports and presentations sound much more convincing if they appeal to the feelings of the audience, and not just to logic. This has to go in: use your own emotions to appeal to other people's emotions.

Use your intuition

We are too often interested in facts and do not ask about premonitions. Facts are pure logic. In contrast, intuition is associated with the right, creative, hemisphere of the brain. The combination of logic and intuition has enormous power.

Apparently, a person makes the most important decisions in his life with the participation of both hemispheres of the brain, and not just the left. However, none of them are usually accepted based on logic, analysis, and facts alone.

The issue of starting a family is not a dispassionate decision based on hard facts and logic. The choice of profession is largely based on feelings, a few signals, as well as impressions and inclinations, but not on rational analysis. The house you live in is also chosen based on feelings, not dry facts - you often act illogically, overpaying to live in a place you like.

Gymnastics (charging) for the neurobic brain
